  1. I agree to abide by the rules, regulations and policies of Merrylands Amateur Swimming Club, Swimming Metro South West, Swimming NSW and Swimming Australia, including Swimming Australia's Anti-Doping, Member Protection, Child Protection and Privacy Policies available at http://www.swimming.org.au/policies-and-procedures.html

  2. I authorise Merrylands Amateur Swimming Club and Swimming NSW to use and disclose to related and relevant bodies any of my personal information that may be necessary to implement the rules, regulations and policies in 1. above. I agree to have my name, photograph and results published in official programs, newsletters and websites.

  3. I understand that as per the Swimming NSW Refund Policy, refunds will not be provided for membership fees. The Swimming NSW refund policy is available at http://nsw.swimming.org.au/page.php?id=143

  4. I understand that my application for membership of Merrylands Amateur Swimming Club is subject to acceptance by the Committee. The Committee may accept or reject an application and shall not be required or compelled to provide any reason for such acceptance or rejection.


Child Welfare Declaration


By submitting this application for membership, I sincerely declare:



  1. I do not have any criminal charge pending before the courts.

  2. I do not have any criminal convictions or findings of guilt for sexual offences, offences related to Children or acts of violence.

  3. I have not had any disciplinary proceedings brought against me by an employer, sporting organisation or similar body involving Child abuse, sexual misconduct or harassment, other forms of harassment or acts of violence.

  4. I am not currently serving a sanction for an anti-doping rule violation under an ASADA approved anti-doping policy applicable to me.

  5. I will not participate in, facilitate or encourage any practice prohibited by the World Anti-Doping Agency Code or any other ASADA approved anti-doping policy applicable to me.

  6. To my knowledge there is no other matter that Swimming Australia Limited may consider to constitute a risk to its members, employees, volunteers, athletes or reputation by engaging me.

  7. I will notify the CEO of the organisation(s) engaging me immediately upon becoming aware that any of the matters set out in clauses above has changed.

  8. I adhere to any state or territory working with Children legislative requirements.


Working With Children Check


Under the Child Protection (Working With Children) Act 2012, paid and un-paid workers in child-related work must be identified, and require a verified Working with Children Check. The club has identified members in the following roles as being required to complete this check and provide their WWC number to the club:



  • Learn to Swim Teachers

  • Swimming Coaches

New South Wales (State)

I agree to abide by the rules, regulations and policies of Swimming NSW (SNSW), Swimming Australia (SAL), the relevant Area Swimming Association and the relevant club, including Swimming Australia's Anti-Doping, and Safe Sport Framework (these are available at www.swimming.org.au).


I authorise Swimming NSW to use, disclose or relate to any relevant bodies any of my personal information that may be necessary to implement the rules, regulations and policies in 1 above. I agree to have my name, photograph and results published in official programs, newsletters and websites.


I understand that the personal information I have provided in my membership application is collected, used and disclosed in accordance with the SNSW Privacy Policy (available from www.nsw.swimming.org.au).  SNSW may use and disclose my personal information for the purposes of conducting and administering swimming and other related activities across New South Wales, providing me with member services or promotional material, complying with legal obligations or otherwise in accordance with the SNSW Privacy Policy. SNSW may share my information with third parties such as:



  1. Swimming Australia (including for direct marketing purposes), SNSW Affiliates and other organisations involved in swimming in New South Wales;

  2. companies engaged by SNSW to carry out functions and activities on their behalf including direct marketing;

  3. government agencies; and

  4. SNSW's professional advisers, including their accountants, auditors, lawyers and insurers,


however my information is not generally disclosed to anyone outside Australia. (The SNSW Privacy Policy contains information about how you may access and request correction of your personal information held by SNSW or make a complaint about the handling of my personal information, and provides information about how a complaint will be dealt with by SNSW.  Your membership application may be rejected if the information is not provided. If you do not wish to receive promotional material from SNSW's sponsors and third parties you must advise SNSW via email or telephone or via the specific opt-out procedures provided in the relevant communication.)


Where the applicant is under 18 years of age this form must be endorsed by the applicant's parent or legal guardian.

If under 18, I as the parent or guardian of the applicant expressly agree to be responsible for the applicant's behaviour and agree to personally accept the conditions set out in this membership application and declaration.
